Description

A £10 charge applies for replacing lost or damaged SHU cards.

 

Detailed Description

Please make your payment, then complete the Student SHU Card Application Form to request a replacement.  You will be prompted to enter your order number on the form. This represents proof of payment and is needed to complete your application for a replacement SHU Card.

A replacement card will only be produced once payment has been made and an Application Form has been completed.

When completing the Student SHU Card Application form students must be logged into Microsoft Services using their student email address in the format of studentID@hallam.shu.ac.uk to access the form.
